Revised: I have serious concerns about the quality of care and oversight at Mosaic in Norfolk, NE, based on my direct experience supporting someone who received services there. Over an extended period, I observed repeated hygiene issues, lack of communication supports, and minimal encouragement toward independence. There were multiple occasions when basic needs were not met promptly, and my repeated attempts to address these issues with management did not result in lasting improvements. In my opinion, the environment did not feel empowering or person-centered, and my concerns were often met with dismissiveness. I also experienced what felt like pushback after raising my concerns through formal channels. Families and caregivers considering services should carefully evaluate options, ask detailed questions, and ensure that their loved one’s needs and rights will be prioritized. My experience left me feeling deeply concerned and hoping that oversight agencies will review practices to ensure individuals are treated with dignity and respect.
Please Read closely before deciding to sending your loved ones to this facility. The day service in Norfolk NE has repeatedly sent clients home saturated in urine and feces. It is astonishing how they can believe they are making a difference in their clients lives. When in all reality they are neglecting them…There also has been incidents reported of their DSP’s (Direct Support Professionals - Day service staff) borderline bullying clients. You would think their head quarters would notice there is a problem when the number of clients have dropped immensely since opening from 52 to 11. Those number alone should tell you there is a big problem…This is supposed to be an environment for individuals to learn and grow but instead they are being mistreated and abused. This is incredibly sad and hurts my heart to see these clients this poorly treated. Mosaic recently had a behavioral specialist come to their facility…during the time she was doing her behavioral analysis Mosaic Staff made sure their clients were not at the facility. Again this is a behavioral specialist her job is quite literally studying behavior. What do you think she was thinking when she showed up and their was hardly any clients there or none at all. This facility is probably the saddest excuse I’ve EVER seen for a day service center.
They don’t treat their people with respect nor there dignity.

In my experience, this organization has repeatedly failed to provide safe and respectful care for the individuals they support. I have personally observed residents left for long periods without assistance, including sitting in soiled clothing. At times, staff have made comments or jokes at the expense of those they are supposed to care for. Many staff spend excessive time on personal cell phones instead of engaging with the individuals, leaving people isolated and unstimulated. When concerns about these issues were raised to management, they were kept internal, with no visible investigation or resolution. I have also seen a lack of meaningful activities and community integration, which are essential for the dignity and development of the people served. In my opinion, these practices reflect neglect and a disregard for the well-being of vulnerable individuals. Families and guardians should be aware of these patterns and ask specific questions about how the organization ensures proper care, respect, and accountability.
