Formal Complaint Regarding Unprofessional Conduct by Jose l. Garcia, NP who works @ Mount Carmel Guild Behavioral Health System • 2201 Bergenline Ave, Union City, NJ To Whom It May Concern, I am writing to formally file a complaint regarding the treatment my mother received from Jose l. Garcia, NP, on August 21, 2025, at Mount Carmel Guild Behavioral Health System. My mother originally had an in-person appointment scheduled with Dr. Jose L. Garcia on August 21. On August 20, she received a confirmation call and informed staff that she would not be able to attend in person but could complete the appointment over the phone, as she had done in the past. Unfortunately, it appears this message was not communicated to Mr. Garcia. When my mother called on August 21 to confirm her telehealth appointment, she was transferred to Mr. Garcia. During this call, Mr. Garcia spoke to her in a highly disrespectful and unprofessional manner. His tone and words suggested she had missed her appointment, even though she had properly rescheduled it the day prior. The call was on speakerphone, and my mother’s homemaker—who was present—was also taken aback by the way he addressed her. The encounter was so upsetting that my mother’s blood pressure spiked dangerously high, reaching 200. This kind of treatment is unacceptable, especially in a healthcare setting where compassion and professionalism are expected at all times. I have since learned that Mr. Garcia is a Nurse Practitioner, not a physician. Regardless of title, his behavior was inappropriate, unprofessional, and potentially harmful to a patient’s health. I am requesting: 1. A formal apology to my mother from Mr. Garcia. 2. That this incident be reviewed internally and addressed with appropriate corrective action. I trust you will handle this matter seriously to ensure no other patient is subjected to such treatment. Sincerely, Juan Alvarez

I've attended Mount Carmel Guild since 2020, graduated the day program in 2022 and have been attending Therapy since 2021. They offer transportation to pick you up and drop you off home, aside from feeding you breakfast & lunch. While I attended the Day Program the group leaders that ran different groups throughout the day while I was there but staff there was never stable, I often kept seeing new faces. There is a nurse that comes in to take blood work as requested by the psychiatrist. Additionally I've seen about 4 different Psychiatrists since starting with Mount Carmel Guild in 2020. However as far as therapy goes, I am beyond thankful to God and to my therapist Rukiya who has been there unconditionally since day one. Personally I had a hard time connecting with a clinician all over NJ since young until 2021 that changed since I started seeing Rukiya. In my opinion, I believe Mount Carmel Guild strives to help their clients as much as they should work on their stability in finding doctors, group leaders, nurses is inconsistent. I feel their patients including myself, would feel more at ease with their wellness journey in seeing someone stable rather than wondering till when you will get a new psychiatrist again or feeling like you are just another number to them to obtain a paycheck from your insurance.

I had a negative experience with one of the clients. The patient advocate didn't address this matter. Gave me the runaround. I don't recommend this agency. Toxic environment!!!! Negative environment

sorry to all my friends from this place but the truth is that everytime I eat here, I get sick or feel a cold coming on. I think this is because the food is out in the open in the cafeteria and the old people that eat there are usually coughing and sneezing with boogers flying. I'm sorry but this is disgusting, the food tastes good but I feel that someone is doing this on purpose to defile and corrupt the food. In addition, many of the clients that come to this place are racists and this could be the real reason why I almost always get sick after eating in this place.
