Consistent attendance is required for effective treatment. Families agree to
maintain at least 90% attendance of scheduled sessions each month.
Excessive absences interfere with treatment integrity and staffing stability. If attendance
drops below 90%, services may be reduced, placed on hold, or discontinued.
Advanced Cancellation Policy
Sessions canceled with at least 24 hours’ notice are considered advanced cancellations.
Families are encouraged to provide as much notice as possible to allow staffing
adjustments.
Same Day Cancellation Policy
Same-day cancellations significantly impact staff scheduling and treatment continuity.
Repeated same-day cancellations may result in service review and potential discharge.
Excessive Cancellations & Fees
Planned absences should be communicated to the Provider as soon as possible. If
cancellations become excessive or interfere with clinical progress:
• Services may be modified or discontinued
• A cancellation fee may be assessed if permitted by funding source and state law
Extended Absences
Families must provide advance notice of vacations. Any absence beyond two weeks is
considered an extended absence. Extended absences may affect service continuation
and may result in reassignment of staff.
Sick Policy
Clients who are ill should not receive services. Sessions must be canceled if the client
has:
• Fever above 100 degrees
• Vomiting or diarrhea
• Contagious illness
• Severe respiratory symptoms
• Eye Infection
• Rash, lice, or nits
• Client was too sick to attend school
• Client is unable to stay awake
Clients must be free of the symptoms listed above for at least 24 hours without the
use of medication intended to mask symptoms before sessions may resume. The
Provider reserves the right to request medical clearance from the client’s physician prior
to restarting services when clinically appropriate. The Provider may request a doctor’s
letter to excuse excessive absences related to illness or medical emergencies.
Transportation is provided solely to facilitate approved ABA treatment activities and is not intended to function as a general transportation service.
• Transportation may occur to and from community-based instructional activities, social skills outings, recreational activities with therapeutic objectives, or other locations identified within the treatment plan.
• Transportation will only be provided by staff who possess a valid driver's license and maintain required automobile insurance.
• Reasonable efforts will be made to ensure the safety of your child during transportation.
• Seat belts and age-appropriate safety restraints will be utilized in accordance with Utah law.
• Staff may transport other ORCA ABA clients when clinically appropriate and authorized by their respective parent(s)/guardian(s).
